Referral Program begins in January 1, 2010. Gift cards can be used towards services and products at the office of Dr Peter B Amundson, including copays and must be redeemed in 2010. The gift card cannot be used in conjunction to any other discounts or offers. Gift cards will be mailed to the referrer within 30 days of referral. The new patient will receive their gift card once they establish themselves as new patients at the office of Dr Peter B Amundson (emergency visits not include) and will receive the gift card at the New Patient visit. Referrals will be documented at Dr Amundson's office with every new patient. Dr Peter B Amundson has the right to suspend the Refer a Friend Program at any time.
